I think it depends on how you qualify it. If it's 1,000 years of excellent health, or maybe only having shitty health in the last 100, I'll take the offer.

Dude that's what plastic surgery is for. And if you can be in tip-top shape for 1,000 years, I don't think you'll look exactly like Swamp Thing.
